FAMILY PEACE CENTER - ILLINOIS CRIMINAL JUSTICE INFORMATION AUTHORITY (ICJIA) MULTI-DISCIPLINARY TEAM PROJECT MANAGER  

        

NATURE OF WORK                


This position serves as the Project Manager for the Multi-Disciplinary Team (MDT) Grant. The Project Manager will be responsible for the management and oversight of the ICJIA MDT Grant awarded to the City of Rockford. The Project Manager will also provide direct services to survivors of domestic and sexual violence and arrange or provide a range of direct individual services to meet the immediate and long-term needs of clients.


GRANT FUNDING ADVISEMENT    

This position is 100% funded by a grant from the Illinois Criminal Justice Information Authority (ICJIA). Grant funding for the current grant period expires in December 2026, with an additional two-year renewal application. Acceptance of this position shall not be construed as a guarantee of employment beyond the grant-funded period.  


DISTINGUISHING FEATURES


The Project Manager will be responsible for management and deliverables of the MDT Grant (“Grant”) as awarded to the City of Rockford, including submission of required data and fiscal reports in a timely manner. The Project Manager will be programmatic reporting on a city and state level and ensuring compliance with grant award conditions.  Work involves significant coordination across multiple City departments, government entities, not-for-profit organizations and community wide organizations to complete the goals of the Grant.  


This position will also dedicate their time to undertaking assessment, monitoring, planning, advocacy and linking with long-term case management, rehabilitation and support services that specifically address the needs of FPC clients who are involved with multiple systems, including the criminal justice system. 


ESSENTIAL FUNCTIONS (These essential duties are only illustrative.)


Completes grant goals and objectives according to proposed timelines, including: ensuring project partners are adhering to trauma informed and survivor driven principles and policies of the FPC and engaging in activities and practices addressing self-care and vicarious trauma.


Conducts training and capacity building for staff and project partners; conducts regular debriefs with staff; in collaboration with the Client Services Manager, updates or recommend changes to all forms used during client intake and case management.


Provides feedback to the FPC Executive Director about additional services, partners or people who need to be at the FPC in order to fulfill client needs.


Serve as intake back-up and provide crisis response and direct services;


Provide outreach to the public and other agencies as requested;


Assist with special projects to further the values of the FPC and objectives of the MDT grant as requested by the FPC Executive Director.


Ensures coordination coverage of service providers and agencies to ensure clients are receiving the assistance they need in an impactful, timely and efficient manner; provides outreach to the public and other agencies as requested.

 

Prepares all grant reports and fiscal reporting for both the City as the Primary recipient and manage sub-awardee reporting.


Works with the City’s Grant Compliance Officer, guided by the City of Rockford Grant Policy to successfully monitor the sub-awardee’s part of the program, project management, and fiscal accountability.


Maintain records, and submit monthly statistical information and evaluating services provided by funded staff and project partners.


Provide long term case management to FPC clients while adhering to trauma informed and survivor driven principles and policies of the FPC.


Treat all clients, staff and FPC partner agency personnel with kindness, dignity and respect; engaging in activities and practices addressing self-care and vicarious trauma. 


Ensure the confidentiality and information sharing process is followed.


Work with clients to develop and maintain relevant and meaningful service plans, which include addressing employment, wellness, health, housing advocacy, public benefits, counseling and court accompaniment support. 


Case Manager will meet regularly with clients to support plan accomplishment, refer clients to other agencies and resources, advocate directly with other agencies as needed to help client access necessary services and accompany clients to services and appointments to support the successful implementation of the case management plan.  Case Manager will also ensure clients have an understanding and knowledge of resource options and facilitate connections and enrollment in those resources.


Participate in training and regular debriefs with the Training and Care Coordinator/Project Manager.


Completes all other tasks and duties as assigned and required by the Manager of the Mayor’s Office of Domestic Violence and Human Trafficking Prevention or Executive Director.


SUPERVISION RECEIVED


Works under the general and day-to-day supervision and direction of the Manager of the Mayor’s Office of Domestic and Community Violence Prevention, who reviews work for effectiveness, timeliness, and for overall results achieved.


SUPERVISION EXERCISED

This position does not require supervisory duties. 


WORKING CONDITIONS & PHYSICAL DEMANDS


Work is performed primarily in an office setting, although work activities require attending meetings and conferences throughout the city and state, visiting multiple locations and inspections on occasion.


SUCCESS FACTORS (KSAs)


Thorough knowledge of domestic violence and human trafficking prevention efforts, including best practice prevention efforts and strategies. 


Comfortable working in a collaborative manner in a fast-paced environment with multiple organizations.  Sensitivity to the concerns of partner organizations, willingness to receive and use feedback in program design, interpersonal interactions and policy development. 


Ability to direct and manage a diverse and multidisciplinary group of individuals, which crosses multiple departments and organizations.  


Ability to express ideas effectively orally and in writing, to conduct research, and to prepare clear and concise reports.

EDUCATION, TRAINING & EXPERIENCE


Graduation from an accredited college or university with a Bachelor Degree in human services, social services or the equivalent, and three years’ experience in case management of persons experiencing trauma and/or mental health concerns, 40-hour ICADV Domestic Violence Training and other training in confidentiality and mandated reporting and have knowledge of the Family Justice Center movement and the purpose of this model and service delivery.  Knowledge of best practice interventions for persons in trauma from domestic or sexual violence, child or elder abuse or human trafficking. Experience with computer programs, data collection desired.  Persons with lived experience with domestic or sexual violence, child abuse or human trafficking preferred. Any satisfactory equivalent combination of education, experience and training, which ensures the ability to perform the work, may be substituted for the required experience.


NECESSARY SPECIAL REQUIREMENTS


Possession of a valid Illinois driver’s license.


Residency Requirement: 


All employees (except Police and Fire) shall reside anywhere in Winnebago County or anywhere within fifteen (15) miles of the Rockford City Hall within six (6) months of completion of their introductory period, department heads and City Administrator shall live within the municipal boundaries of the City of Rockford within six (6) months of their completion of their introductory period.
