Satori Pathway Memory Resource Network, Rockford, is designed to help individuals, families, physicians, discharge planners and healthcare organizations. Satori Pathway has two levels of service: 1. to help families create individualized plans and navigate systems of care to provide services for individuals/couples and their families with respect to a dementia, and 2. to provide consulting and support services to help owners and operators of healthcare facilities working with persons who have a dementia to establish and maintain a state-of-the-art, person-centered care program, allowing the person(s) with the disease to stay in the home of choice as long as possible, in most cases. Satori Pathway will develop a plan specific to an individual's needs. It will keep in close contact with families and physicians, monitoring and supporting the situation, and adjusting plans as needs change. The people behind the Satori program have more than five decades of direct experience in assisted living, home-based services, geriatric psychiatry and medicine, dementia care, memory care, and in navigating our complex health care system. Satori Pathway operates Anam Care and Anam Glen, on-site Rockford resident facilities which are built around the Anam Cara philosophy of palliative dementia care. They focus on compassionate care, comfort, dignity and life. Satori's programs meet the specialized needs of all who are afflicted with a dementia, including Alzheimer's, Lewy Body, Parkinson's, Frontal-temporal dementias, and other forms of dementia or cognitive disorder. The steps to determine the need and level of services are as follows (at no charge): 1. One-hour in-home assessment. 2. Counseling about appropriate services. 3. Follow-up support call to potential resident's or client's family or specified contact. 4. Identification of personal, professional contact at Satori Pathway.
